what would be the reason notifications to the respective party are getting multiple emails?

I have a termination custom command that will disable acct, reset pwd, move acct to OU and set expiration date.
if its voluntary or involuntary, it goes to a group of ppl.

the recipients are stating they are getting multiple emails and on the home screen it says "saving".
